Palliative Care

Have you or a loved one just returned home from the hospital and still need help with things that you can't do on your own? Our palliative services can assist!

If you answer yes to at least two of the following, you might be appropriate for palliative care:

​

  • Are you afraid that you won't be able to get to the doctor on time, or you don't want to go back?

  • Have you had 2 or more recent hospitalizations?

  • Do you have 2 or more chronic conditions, like COPD or CHF?

  • Have you had 2 or more infections in the last 6 months that required antibiotics?

What is provided?

Palliative Care provides assistance getting local resources in the home. This can include:

  • grocery deliveries

  • assistance with food programs like Meals on Wheels

  • nursing or social work visit once a month

  • coordinating in-home doctors’ visits

  • help organizing medication deliveries

  • advice, referrals, and assistance with advanced directives like living wills, power of attorney paperwork and more.
