If you answer yes to at least two of the following, you might be appropriate for palliative care:

Are you afraid that you won't be able to get to the doctor on time, or you don't want to go back?

Have you had 2 or more recent hospitalizations?

Do you have 2 or more chronic conditions, like COPD or CHF?

Have you had 2 or more infections in the last 6 months that required antibiotics?

What is provided?
Palliative Care provides assistance getting local resources in the home. This can include:

grocery deliveries
-
assistance with food programs like Meals on Wheels

nursing or social work visit once a month
-
coordinating in-home doctors’ visits
-
help organizing medication deliveries
-
advice, referrals, and assistance with advanced directives like living wills, power of attorney paperwork and more.
